Please complete this form if your student was absent on Fall Picture Day or if you would like their photo remade.

Absent or New Students:
You may submit your student’s name, but it is not required. We receive a list of absent and new students directly from the school.

Remake Requests:
If your student had their photo taken and you would like a remake, please return the original picture package or proof to the school on Remake Day. This lets us know that you would like the original order reprinted using the new photo. If the original package is not returned, we will not reprint the package.

Please let us know the reason for the remake so our photographer knows what to look out for and can help make the new photo successful.
